General Information

Spring Branch Community Health Center is a full-service health center that addresses all the basic health care needs of our patients, including medical, dental, behavioral health, and health education services. Not all services are offered at every location, so make sure that you confirm that the service you are seeking is available at the location nearest you.

In addition to accepting Medicaid, Medicare and Private health insurance plans, we also serve individuals that are not covered by health insurance. Patients are charged for services on a sliding fee scale based on the patient’s family size and income. In accordance with Federal regulations, proof of income must be provided for the patient to be eligible for any sliding fee discount on services. Spring Branch Community Health Center is an FTCA (Federal Tort Claims Act) deemed facility.

Eligibility Assistance

An Eligibility Specialist is available to assist with enrollment in Medicaid, and CHIP, and Gold Card Applications for specialty services. Patients can apply for Medicaid and other public insurance programs onsite at both of our locations.

Needs-Based Consideration

We also work with patients on an individual basis to setup payment plans, if they have personal financial situations where patient co-pays cannot be made at the time of service.

Financial Assistance Program

The Financial Assistance Program is a special program to assist those who are uninsured or have difficulty paying for medical care. If you are not eligible for insurance coverage and have limited income, you can apply for a sliding fee discount.

Patients who are not insured or who may qualify for a sliding fee discount, Medicaid or other assistance programs should bring proper identification, proof of income and other supporting documents.
